We offer immediate availability for telehealth services and can provide face-to-face appointments when necessary. Our support coordination services are designed to cater to complex clients, including those needing specialised community care and disability support services, such as forensic support.
TIDE Connect provides professional Level 2 Support Coordination, Level 3 Specialist Support Coordination and psychosocial counselling tailored to the individual needs, circumstances and goals of NDIS participants.
We work collaboratively with participants, families, carers, informal supports and service providers to navigate the NDIS, understand available funding, coordinate appropriate services and establish reliable support networks. Our focus is on strengthening each participant’s capacity to make informed decisions, exercise choice and control, and use their NDIS plan effectively.
We have extensive experience supporting people living with mental health conditions, psychosocial disability, acquired brain injury, intellectual disability, physical disability, complex support needs and behaviours of concern. We understand that these circumstances may involve multiple services, elevated risks, changing needs and barriers to accessing appropriate care and support.
Our areas of expertise include crisis and contingency planning, risk assessment and management, hospital discharge coordination, housing and accommodation pathways, safeguarding, multidisciplinary service coordination and advocacy. We work proactively to identify service gaps, clarify responsibilities, reduce risk and establish sustainable support arrangements that promote safety, stability and wellbeing.
Our approach is person-centred, trauma-informed, strengths-based and responsive. We recognise that every participant’s circumstances are different and that effective support coordination requires more than simply connecting a person with providers. It requires careful planning, consistent communication, practical problem-solving and ongoing oversight of how supports are working together.
Psychosocial counselling is also available to support participants experiencing emotional distress, adjustment difficulties, complex life circumstances or barriers affecting their wellbeing and engagement. Counselling is tailored to the participant’s needs and delivered within the practitioner’s professional scope, with referrals to appropriate clinical or crisis services when required.
Whether a participant requires assistance navigating the NDIS, coordinating multiple providers, responding to a significant change in circumstances or stabilising a complex support environment, TIDE Connect remains focused on practical and meaningful outcomes.
